Ascending cholangitis is a severe infection of the biliary tree, commonly caused by ductal obstruction, most often due to choledocholithiasis. This condition requires prompt diagnosis and management because of its high morbidity and mortality if left untreated. This case analysis explores the clinical progression of a patient with severe ascending cholangitis, emphasizing the major multisystem complications that may arise, including sepsis, liver failure, and renal dysfunction. Diagnostic criteria, the critical role of imaging, and the importance of a multidisciplinary treatment approach are discussed. This includes broad-spectrum antibiotics, urgent biliary drainage, and intensive life support. The case highlights the necessity of early intervention and continuous monitoring to prevent fatal outcomes, serving as a valuable reference for the clinical management of similar hospital scenarios.
