Diagnóstico y manejo de la neoplasia linfoproliferativa ovárica en paciente pediátrica: Análisis de caso

Abstract

Primary ovarian lymphoma is a rare neoplasm, accounting for only 1-1.5% of ovarian cancers. Among these, Burkitt lymphoma is an aggressive and rapidly growing variant, more common in childhood. Due to its rarity, diagnosis may be delayed, affecting prognosis. This study presents the case of a 5-year-old patient with bilateral Burkitt-type ovarian lymphoma, highlighting its clinical characteristics, diagnosis, and treatment. A systematic review of the literature in indexed databases (2015-2024) was conducted, including a bibliometric analysis and a detailed study of the patient's medical history. Imaging, histopathological, and immunohistochemical studies were used to confirm the diagnosis. Staging was performed according to FIGO criteria and the Ziegler and Magrath system. The patient presented with abdominal distension and a palpable ovarian mass. Exploratory laparotomy revealed bilateral tumors with hemorrhagic secretion and peritoneal carcinomatosis. Histopathological analysis showed the characteristic "starry sky" pattern of Burkitt lymphoma. Immunohistochemistry confirmed the overexpression of CD20, CD19, and Ki67 (>95%). The case was classified as stage 1-A (FIGO) and stage C (Ziegler and Magrath). The patient received chemotherapy and showed a favorable outcome. Burkitt-type primary ovarian lymphoma should be considered in pediatric patients with ovarian masses and nonspecific symptoms. Early detection, immunohistochemical diagnosis, and timely treatment are important to improving prognosis and survival in these cases.
