Síndrome de DRESS en el paciente quirúrgico: diagnóstico diferencial, manejo perioperatorio y desafíos terapéuticos

Resumen

El síndrome de reacción a fármacos con eosinofilia y síntomas sistémicos (DRESS) constituye una reacción adversa grave asociada al uso de determinados medicamentos y representa un desafío particular en el contexto perioperatorio debido a la polifarmacia, la complejidad clínica y la posible superposición con otras complicaciones. El objetivo de esta revisión fue caracterizar el síndrome DRESS en pacientes quirúrgicos, con énfasis en su reconocimiento y abordaje terapéutico. Se realizó una revisión bibliográfica que incluyó 25 referencias publicadas durante los últimos diez años en español, inglés y portugués. La evidencia analizada muestra que el reconocimiento precoz del síndrome, la identificación y suspensión inmediata del medicamento desencadenante y el inicio oportuno de corticoterapia continúan siendo los principales pilares del tratamiento. En casos graves o refractarios se han empleado alternativas terapéuticas como ciclosporina, inmunoglobulina intravenosa, plasmaféresis y determinados agentes biológicos dirigidos contra mecanismos relacionados con los linfocitos T y las interleuquinas. Sin embargo, estas intervenciones presentan evidencia limitada y carecen todavía de protocolos suficientemente estandarizados. En el paciente quirúrgico, el abordaje requiere individualizar las intervenciones, proporcionar soporte general, mantener una monitorización estrecha y tratar oportunamente las complicaciones agudas. Se concluye que el DRESS representa una reacción de hipersensibilidad potencialmente grave cuya presentación perioperatoria plantea importantes dificultades diagnósticas y terapéuticas, especialmente por el riesgo de afectación multiorgánica. Aunque las herramientas diagnósticas disponibles y la corticoterapia temprana favorecen su manejo, persisten importantes vacíos de conocimiento que justifican investigaciones con poblaciones más amplias y metodologías que permitan establecer estrategias terapéuticas específicas para el contexto perioperatorio.
