Agonistas del receptor GLP-1 en enfermedades reumáticas inflamatorias: impacto inmunometabólico, obesidad y potencial terapéutico

Resumen

Las enfermedades reumáticas inflamatorias (ERI) constituyen una de las principales causas de discapacidad a nivel mundial y se caracterizan por un estado persistente de activación inmunológica asociado con inflamación crónica. En este contexto, el presente estudio tuvo como objetivo analizar el potencial terapéutico de los agonistas del receptor del péptido similar al glucagón tipo 1 (GLP-1) en las enfermedades reumáticas inflamatorias, con especial énfasis en su impacto inmunometabólico y en su utilidad en pacientes con comorbilidades metabólicas, particularmente la obesidad. Para ello, se realizó una revisión bibliográfica de la literatura científica publicada en español e inglés. La evidencia revisada muestra que las ERI comparten mecanismos fisiopatológicos relacionados con la pérdida de la tolerancia inmunológica, la producción de autoanticuerpos, la formación de inmunocomplejos y la inflamación sistémica. Asimismo, se identificó que trastornos metabólicos como la obesidad, la diabetes mellitus tipo 2 y las dislipidemias favorecen un estado inflamatorio de bajo grado que incrementa la actividad clínica y el peor pronóstico de estas enfermedades. Los estudios analizados sugieren que los agonistas del receptor GLP-1 poseen efectos antiinflamatorios e inmunometabólicos que podrían complementar el tratamiento convencional, al mejorar el control metabólico y reducir la inflamación sistémica. Aunque los resultados disponibles son prometedores, la evidencia aún es limitada, por lo que se requieren estudios clínicos con mayor rigor metodológico que permitan confirmar su eficacia y seguridad en el manejo integral de las enfermedades reumáticas inflamatorias.
