Benefits and barriers of advance palliative care planning in patients with mild dementia. Systematic review

The study analyzes advance care planning in patients with mild dementia and its impact on quality of care, autonomy, and end-of-life decision-making. A systematic review was conducted using databases such as PubMed, EMBASE, CINAHL, and Cochrane, identifying 185 articles and selecting 28 according to PRISMA guidelines. The results indicate that advance care planning improves patient knowledge, promotes respect for autonomy, increases participation in decision-making, and enhances communication with healthcare professionals, leading to a higher perceived quality of care; however, social, economic, institutional, accessibility, and communication barriers were identified, along with limitations in staff training and difficulties in discussing the topic with patients and families. In conclusion, advance care planning is beneficial, as it supports autonomy, knowledge, and decision-making, contributing to improved quality of palliative care, and should therefore be implemented in a personalized manner centered on the patient and their family.
