Therapeutic options for odontogenic keratocyst: a systematic review using Prisma 2020 methodology

Odontogenic keratocysts (OKCs) are chemical lesions of odontogenic origin with aggressive behavior and a high recurrence rate, posing a challenge in their diagnosis and treatment. Currently, there is no consensus on the best therapeutic strategy, as treatment choice depends on factors such as lesion size, location, and relationship with adjacent anatomical structures. The objective of this study was to analyze, through a systematic review based on the PRISMA 2020 methodology, the different therapeutic options available for the management of OKCs and their impact on recurrence rate. A bibliographic search was conducted in PubMed, Medigraphic, SciELO, and Google Scholar, including studies published between 2019 and January 2023, with MeSH/DeCS terms related to OKCs and treatment. Thirteen articles were selected after applying inclusion and exclusion criteria. The results indicated that simple enucleation presents a high recurrence rate (32.9%), while block resection shows the lowest recurrence rate (0%). Decompression followed by enucleation, combined with adjuvant therapies such as Carnoy's solution and cryotherapy, has shown to reduce recurrence and preserve mandibular functionality. It is concluded that, although conservative strategies with adjuvants offer good results, there is no standard protocol and controlled clinical trials with long-term follow-up are required to establish optimal treatment and reduce uncertainty in clinical decision-making.
