Therapeutic adherence plays a crucial role in the effectiveness of medical treatments and the management of chronic diseases. However, non-adherence remains a significant public health challenge with wide-ranging clinical and social implications. This study aims to analyze the influence of personality on treatment adherence, contributing additional data that highlight the importance of psychological factors in following clinical therapies. The research combined a qualitative approach, through a systematic review of scientific literature based on the Five-Factor Model, with a cross-sectional quantitative analysis conducted on 30 patients with chronic illnesses and neuropsychiatric disorders. The study examined how personality traits affect therapeutic adherence, using a multiple linear regression model to identify relevant patterns according to clinical diagnosis and psychological profile. Responsibility was found to enhance treatment compliance, while neuroticism and impulsivity hinder it. Furthermore, variations in adherence were observed among patients with borderline personality disorder, chronic conditions such as diabetes, and adolescents with ADHD, underscoring the need to tailor medical interventions to each patient’s psychological characteristics. Cross-cutting strategies were proposed to improve therapeutic adherence across diverse clinical settings.
